Royal,    I believe that if it will be your first weigh-in of the year and
your membership is still valid, you will only need the transfer form from the
old chapter, no need for the yellow weight chart.  Of course then the transfer
form will be sent in by your new chapter.   If you happened to be renewing at
the time, you don't even need the transfer form.
